- Any accident, fall or whiplash in your history
- Whether you are in Malaysia or Singapore, and your city
- Preferred dates if you are travelling
What to bring to the appointment
- Comfortable clothing
- Any relevant scans, reports or a list of medication
- Your insoles, if you wear them, so leg length can be assessed properly
- For children under 16: a parent or legal guardian
